As part of the employment process, please attest to the following:
I understand that if I apply and if I am interviewed, that I will be asked to authorize a pre-employment investigation which would only be conducted after an offer of employment has been made to me. The pre-employment investigation may include, but is not limited to, a pre-placement job related physical exam, alcohol and other drug screen, criminal history check, educational and work history verification, excluded and sanctioned person check and any other investigation required by local, state, or federal law. I understand that misrepresentation or omission of information in connection with my application and/or interview will be sufficient cause in and of itself, at Medica's sole discretion, for rejection of my application or dismissal from employment whenever discovered. I understand that any offer of employment is dependent upon the satisfactory completion of the above-mentioned investigations and that the investigations could also occur during the course of my employment.
I understand that if I am hired by Medica, my employment will be for an indefinite period of time and will be "at will" which means either I or Medica may terminate the employment relationship at any time and for any reason. I further understand that, if hired, my at-will employment status may only be changed in a written contract signed by an authorized representative of Medica, and that no unauthorized representative of Medica has the authority to make any promise to me concerning my employment. Finally, I also understand that Medica's policies and benefits may change at any time, with or without notice to me.
